Birth & Death Certificates

Vital Records

The Carroll County General Health District provides certified birth and death certificates. The cost is $27.00 for each certified copy.

Birth and Death Certificates are issued Monday, Tuesday, Wednesday, and Friday from 8:00 AM and 4:00 PM, and Thursday's from 8:00 AM to 12:00 PM. 



CCGHD Offers Online Ordering for Birth and Death Certificates

Online ordering is available to purchase a certified birth and death certificate. All orders placed online can either be picked-up at the health department or mailed. Customers will be updated on the status of their online order by email when it is ready or if additional information is needed to process the order. 

A credit card payment is required to process all online orders. An additional convenience fee is charged for this service.

Birth Certificates 

  • The office only issues certificates for births that occurred in Ohio from December 1908 – present. 
  • For births outside the State of Ohio, please contact the state in which the event occurred. 
  • Records prior to 1908 are located at the Carroll County Genealogical Society in the basement of the Carroll County Library by using the back entrance at 70 2nd St. N.E., Carrollton, OH 44615. Phone number is 330-627-9411.

New Birth Certificates 

  • Certificates for new births typically take up to 2 weeks to become available. While the Carroll County General Health District Office strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into the State of Ohio records system.

 

Death Certificates 

  • This office only issues certificates for deaths that occurred in Carroll County, Ohio from December 1908 – present.
  • Records prior to 1908 are located at the Carroll County Genealogical Society in the basement of the Carroll County Library by using the back entrance at 70 2nd St. N.E., Carrollton, OH 44615. Phone number is 330-627-9411.
  • For deaths outside the State of Ohio, please contact the state in which the event occurred. 
  • For deaths occurring outside Carroll County (example: hospital), but within Ohio, contact the county's office of vital statistics where the death occurred or the Ohio Department of Health.   

Social Security Numbers on Death Certificates

Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent’s death, a decedent’s social security number shall not be included on a certified copy of the decedent’s death certificate unless that information is specifically requested to be on the certified copy. Relationship restrictions apply and acceptable proof is required. 

Those relationships able to receive a death certificate WITH the decedent’s social security number include: 

  • The deceased's spouse or lineal descendant
  • The deceased's executor, attorney, or legal agent
  • A representative of an investigative government agency
  • A private investigator
  • A funeral director (or agent responsible for disposition of the body) acting on behalf of the deceased's family
  • A veteran's service officer
  • An accredited member of the media

Acceptable/Satisfactory Proof of Relationship:

  • Current State Issued Photo Identification 
  • Birth certificate of Natural or Adopted Child (and marriage/divorce documents if last name has changed)
  • Legal document issued through a US court of law authorizing a person to act on behalf of the decedent or the decedent’s estate 
  • Marriage license of spouse or legal partner (not needed if the decedent’s Death Certificate designates the name of the surviving spouse)  
  • Business card, license to practice, and/or employee photo ID
